Preparing for adulthood is a crucial aspect of SEND support, focusing on equipping young people with SEND with the skills and knowledge needed for successful transition into adulthood. This involves four key areas: further education or employment, independent living, community participation, and good health. The SEND Code of Practice emphasises the importance of early planning and collaboration between education, health, and social care services to support young people's aspirations.

In practice, preparing for adulthood means preparing for:

  • Higher education and/or employment – including exploring different employment options
  • Independent living – this means young people having choice, control and freedom over their lives and the support they have, their accommodation and living arrangements, including supported living
  • Participating in society – including having friends and supportive relationships, and participating in, and contributing to, the local community
  • Being as healthy as possible in adult life

 Local authorities are legally required to provide suitable support that helps children and young people with Special Educational Needs and Disabilities (SEND) prepare for adulthood.

Person Centred Planning Tools- Person Centred Approaches

Person Centre Approaches; Person centred planning places the young person and their family at the centre of decision making.  It requires the young person to be recognised as an individual with strengths and challenges, likes and dislikes.  Person centred planning tools such as one page profiles, working / not working and important to / important for can help with planning support that reflects the individual needs, strengths and aspirations of the young person and their family.
